The "File of Life" is a miniature medical history that is kept in a red plastic sleeve.  Once the card is filled out it should be placed in the red sleeve and carried on your person at all times.  This card will enable emergency personnel to obtain a quick medical history when a patient is unable to give one.

 

Information on the card that will remain constant such as person's name, date of birth, social security number, allergies, past medical history, etc. should be filled out in pen.

Information that will be subject to change such as medications, dosages, contact numbers, current problems, etc. should be filled out in pencil.

 

Files can be picked up at the Safety Complex and at community events.

 

Donations can be made by donating your Mass. returnables to Redemption Plus on Route 44 in Rehoboth and asking them to put the money in the Seekonk Police @ Fire account #111 or by putting your Mass. returnables in the designated container near the dumpster at the Safety Complex.  Cash donations will also be accepted and checks can be made out of Seekonk Police & Fire Files of Life Campaign.

 

One dollar could save three lives.
